I’m looking forward to meeting you and leave you with beautiful lashes 💕 ***By making an appointment you are automatically agreeing to ALL policies.*** • PLEASE NO EXTRA GUESTS ARE ALLOWED. • NO REFUNDS. • No call/no shows will not be allowed to book with me again and will be charged 100% of the service. Please take my time into consideration as I do with yours. •Refills must have 40% of lashes remaining. Anything less will be considered a full set. •If you do experience major fall out the first two days after your service please let me know within that 48hr timeframe so I can give you a complimentary fix. Remember that the free fill is complied only for the first 2 days after your appointment in order for me to give you a complimentary fill! • If you're sick please let me know in advance and we can figure out a date to reschedule. • I have a 15 minute grace period. Arriving late we will need to reschedule your appointment. • AFTER HOUR FEE: After hour appointments will be charged an additional $25. • HOW TO SHOW UP TO YOUR APPOINTMENT: Please come with clean lashes. The cleaner the lash, the better the retention! - If you wear contact lenses, please be sure to take them off before the start of your appointment. This will prevent you from getting any reactions/irritations. •NO REFUNDS- DUE TO ALLERGIC REACTION/ALLERGIES -If this is your first time getting eyelash extensions, you are doing it at your own risk. Allergic reactions CAN happen and they DO happen If you’ve never gotten them. If a reaction were to occur, I am not responsible. Same rules apply to people who get lashes done regularly but eventually developed an allergic reaction over time. If you do develop an allergic reaction please don’t hesitate in contacting me and I will offer a free lash removal. • I understand that if I have any concerns, I will address these with my lash extension specialist. I give permission to my lash extension specialist to perform the lash extension procedure we have discussed, and will hold him/her and his/her staff harmless and nameless from any liability that may result from this treatment. I understand my lash extension specialist will take every precaution to minimize or eliminate negative reactions as much as possible. In the event I may have additional questions or concerns regarding my treatment, I will consult the lash extension specialist immediately. I agree that this constitutes full disclosure, and that it supersedes any previous verbal or written disclosures. I certify that I have read, and fully understand, the above paragraphs and that I have had sufficient opportunity for discussion to have any questions answered. I understand the procedure and accept the risks. I do not hold the lash extension specialist, responsible for any of my conditions that were present, but not disclosed at the time of this procedure, which may be affected by the treatment performed today.
